- Population general health risks are identified and assessed through a Health Risk Assessment*
- Cardiovascular disease risks are identified by our proprietary HeartAgeTM screening tool
- Lifestyle behavior change tools including healthy eating, exercise, and smoking
- Medication adherence and safety via our robust online tools
The above tools are conveniently incorporated into one online platform, known as the Good Health Gateway® Wellness Program. The structure and incentives of the program are set up in such a way as to reach the individuals who need it the most – those who are either currently untreated or non-adherent to treatment guidelines. Our program both detects and manages risks in the overall population and provides tailored, individual input to members that aims to educate about risk and motivate health behavior change. The online platform is supported by our Call Center, which gives members live access to individuals available to answer questions and facilitate participation in any of our programs.
