Every employer is distinct and so are the needs of their population, but Asthma, Diabetes, Depression, High Blood Pressure (Hypertension), High Cholesterol (Hyperlipidimia), and Prenatal Health are some of the common areas of need of more active management to control costs in employer health plans. It is these conditions and health categories that drive the bulk of the cost of employer health plans. For example, the CDC reported that in 2011 one in three adults in the U.S. have diabetes or pre-diabetes.
Abacus’ chronic condition-specific programs reward members for following clinically-based standard of care guidelines for the management of their condition or conditions. Incentives are customized to fit with each employer, but often we suggest incentives like waived co-pays or free health care supplies (like diabetic test strips). The use of incentives drive participation in our programs, and the use of medical or health plan related incentives increases member access to goods and services they need to stay healthy and productive.
The members with chronic conditions are offered the incentives if they follow proper care for their condition, like going to see their Primary Care Physician (PCP) on a regular basis and following up with important tests and exams. Patient and provider communication is a focal point of all of Abacus’ interventions. The ability to further support the members with specialists, like a Certified Diabetes Educator, is easily added into the program, and that clinical staff can be provided by Abacus, or the program can drive to in-house, on-site recourses if available.
