Abacus is collaborating with the Memorial Hospital of Rhode Island and the Warren Alpert Medical School at Brown University’s Center for Primary Care and Prevention and Project Director Charles Eaton, MD, MS on a new e-Blood Pressure (eBP) control project.  This study, funded by the Agency for Healthcare Research and Quality (www.AHRQ.gov), is developing and evaluating an eHealth enabled Patient-Centered Medical Home (PCMH) model of care for improved hypertension and elevated blood pressure control.  The ongoing study will directly evaluate outcomes for patient activation, self-management activities, medication adherence, reducing clinician inertia, and improved blood pressure management compared to a control group.

Using the technology developed in part by the grant, Abacus has developed a specialty clinic version of the program that can be deployed in hospital settings, such as renal transplant clinics where proper blood pressure management is critical to avoid organ rejection.  The solution engages patients with appropriate incentives and additional clinical support, like regular encounters with the clinical team and other in-house resources, like hospital nurses or pharmacists.  Measures of medication adherence and blood pressure readings are part of the medical data provided to the clinical team to allow them to make the best decision on behalf of the patient and achieve the best health outcomes.  Innovative eHealth technology solutions are integral components of improving quality of care, meaningful use of HIT for providers and patients, and can assist provider organizations in their efforts to become Accountable Care Organizations (ACO).

The Abacus program allows for streamlined blood pressure self-management with individually tailored feedback from the Good Health Gateway® (a proprietary web-based, self-management portal developed by Abacus) using information recorded with a home blood pressure monitoring device that is synchronized with the Microsoft® HealthVaultTM Personal Health Record (PHR) platform as the data repository.  Abacus is in the early phases of integration of data from a Personal Health Record directly into a health system’s Electronic Medical Record (EMR) to allow doctors to make better decisions and provide more seamless care.
